Abstract: This study developed a diagnostic based on plasma extracellular vesicle long RNA (exLR) for the detection of pancreatic ductal adenocarcinoma (PDAC). The study involved 501 participants, including patients with PDAC, chronic pancreatitis, and healthy controls. Through exLR sequencing and machine learning algorithms, researchers identified a diagnostic signature comprising eight exLRs that demonstrated high accuracy in training, internal validation, and external validation cohorts. This diagnostic can effectively identify early resectable PDAC and performs better than the traditional carbohydrate antigen 19-9 test in differentiating PDAC from chronic pancreatitis. This finding may contribute to improving early diagnosis and treatment outcomes for patients with PDAC.
